In Home Community Services
In Home service program is a community-based service program designed to provide in-home support and assistance to individuals with developmental disabilities. Our mission is to promote independence, enhance quality of life, and foster community integration for participants and their families.
We provide personalized care plans in collaboration with the individual, their family, and a team of professionals, tailoring goals to the participant's unique needs, abilities, and preferences.
We provide trained caregivers who assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, meal preparation, feeding, and medication management as required.
We provide a focus on enhancing independent living skills, communication abilities, and social interaction skills through structured activities and exercises designed to promote skill acquisition and retention.
We provide organized outings to local parks, libraries, recreational centers, and cultural events to encourage social inclusion and participation, prioritizing meaningful community engagement opportunities for participants.
We provide educational resources, counseling services, and support groups for family members to address their caregiving roles, recognizing and addressing their needs and concerns.
We provide regular health assessments to monitor physical and mental well-being, collaborating with healthcare professionals to address medical or behavioral concerns promptly.
We provide guidance and support for obtaining adaptive equipment and making necessary modifications to the home environment to enhance accessibility and safety.
